Life is unpredictable. We are not always prepared when tragedy strikes. When it does, it can be overwhelming to be asked to make healthcare decisions for someone whose condition is deteriorating. Nanticoke Health Services is here to assist you in documenting your choices and helping your loved ones honor your wishes.
There are a number of documents that are important to think about, while you are capable. The advance directive form allows you to choose the person (“medical power of attorney”), who can make medical decisions for you. The advance directive is effective only if you become incapacitated. This form is vital in allowing the patient to make end-of-life decisions in writing, so family and friends will know your wishes. You do not need a lawyer to do an advanced directive, and notarizing is optional. Two types of decisions may be needed:
The financial powers of attorney form allows someone to manage money for the person who writes that power of attorney. Sometimes individuals want one person to make medical decisions and another to make financial decisions if they become incapacitated. A person with only financial power of attorney cannot make healthcare decisions for you. Both documents are important, but are different. Remember: legal advice is needed for a financial power of attorney, but not for an advanced directive. Organ donor permission is an additional document to think about if you have the desire to be an organ donor. This can be addressed on your driver’s license, and also on your advance directive form.
